Complex Trauma (C-PTSD) occurs when an individual experiences repeated, prolonged, or chronic traumatic events, typically within interpersonal relationships where there is an expectation of safety, trust, or care. Unlike a single-event trauma (such as a motor vehicle accident or natural disaster), complex trauma is developmental and relational, deeply shaping an individual's nervous system, sense of self, and capacity for connection.

At Wellness Counselling Centre, we provide specialized, trauma-informed care to help you move out of a constant state of survival, process deep emotional wounds, and safely rebuild your life, autonomy, and self-trust.

Understanding the Clinical Presentation of

Complex Trauma

While the DSM-5 classifies trauma-related presentations under Post-Traumatic Stress Disorder (PTSD), clinical consensus and international diagnostic frameworks (such as the ICD-11) recognize Complex PTSD (C-PTSD) as a distinct clinical presentation. It encompasses the core symptoms of PTSD alongside a secondary triad of severe, persistent disturbances in self-organization:

Core PTSD Symptoms:

  • Re-experiencing: Intrusive memories, vivid flashbacks, or distressing nightmares where the trauma feels as though it is occurring in the present moment.

  • Avoidance: Active, persistent efforts to avoid internal triggers (memories, thoughts, or feelings) and external triggers (people, places, or conversations) associated with the trauma.

  • Hyperarousal & Threat Detection: Living in a constant state of high alert, characterized by hypervigilance, an exaggerated startle response, irritability, and sleep disturbances.

Disturbances in Self-Organization (The "Complex" Triad):

  • Severe Emotional Dysregulation: Experiencing intense, rapid mood shifts, chronic emotional numbing, or explosive anger, with a limited capacity to soothe or regulate your own nervous system.

  • Persistent Negative Self-Concept: Carrying chronic feelings of shame, guilt, worthlessness, and a deeply ingrained belief that you are permanently damaged or fundamentally different from others.

  • Profound Relational Impairment: Experiencing persistent difficulties in maintaining relationships, characterized by an intense fear of abandonment, difficulty trusting others, or repeatedly entering unsafe relational dynamics.

Noticing the Impact: How C-PTSD Affects Daily Life

Because complex trauma is often sustained over months or years—such as during childhood abuse, chronic domestic violence, or prolonged exploitation—the brain and body adapt to survive. You may recognize these survival responses in your daily life:

In Yourself:

  • The "Fawn" Response: Automatically placating, pleasing, or merging with others to avoid conflict, criticism, or perceived danger, often at the complete expense of your own boundaries.

  • Dissociation & Depersonalization: Feeling spaced out, emotionally numb, or disconnected from your physical body and reality during times of stress.

  • Chronic Somatic Symptoms: Living with unexplained physical pain, gastrointestinal distress, migraines, or chronic tension, as the body holds the physiological stress of the trauma.

  • Inner Critic Attacks: Navigating an relentless, highly critical internal voice that reinforces feelings of failure, self-blame, and shame.

In Relationships & Families:

  • Boundary Extremes: Alternating between highly rigid boundaries (completely shutting others out to stay safe) and porous boundaries (allowing others to cross your limits out of fear of rejection).

  • Anxious-Avoidant Cycles: Desiring deep emotional closeness but feeling highly anxious, panicky, or suffocated when someone attempts to get close to you.

Healing and Rebuilding: Our Approach to Therapy

Healing from complex trauma requires a structured, phased clinical approach. We prioritize your safety and pacing above all else, ensuring we never rush into processing memories before your nervous system is ready.

  • Phase 1: Safety & Stabilization:

    • We focus on establishing physical and emotional safety, teaching you concrete grounding techniques, and helping you understand your survival responses (fight, flight, freeze, fawn).

    For Children and Youth:

    • We help children and teens understand their worries through creative, age-appropriate tools, building their resilience and helping them face fears gradually.

    Phase 2: Titrated Trauma Processing:

    • Once your nervous system is stabilized, we gently and gradually process traumatic memories in a highly structured, supportive environment, helping to reduce their emotional charge.

    Phase 3: Integration & Reconnection:

    • We focus on reclaiming your identity beyond your trauma, rebuilding your self-worth, establishing healthy boundaries, and fostering safe, fulfilling relationships.

Our Therapeutic Toolkit

To support your recovery, we utilize specialized, neurobiologically-informed modalities designed to address both the cognitive and physical impacts of complex trauma:

  • Somatic & Sensorimotor Psychotherapy: Focuses on how trauma is physically stored in the body, helping you safely track physical sensations, complete interrupted survival responses, and restore a sense of physical safety.

  • Internal Family Systems (IFS): Helps you identify and heal the vulnerable "parts" of your system that carry chronic shame and pain, while working collaboratively with the protective parts that manage your daily survival.

  • Cognitive Processing Therapy (CPT): A highly structured, trauma-focused cognitive therapy that helps you evaluate and modify upsetting, deeply ingrained beliefs (such as issues of safety, trust, control, esteem, and intimacy) that have been disrupted by trauma.

  • Dialectical Behaviour Therapy (DBT) Skills: Teaches concrete distress tolerance, mindfulness, and emotional regulation skills to help you manage intense emotional waves without feeling overwhelmed.

  • Narrative Therapy: Helps you externalize the trauma, allowing you to reframe your personal history, challenge self-blame, and reclaim your voice and agency.

  • Trauma-Informed Cognitive Behavioural Therapy (TF-CBT): Assists in identifying and restructuring deeply held, trauma-related beliefs (such as "I am fundamentally unsafe" or "I am responsible for what happened").

Whatever your are facing, there is a place to start.
